三个法语教育局

• Quebec driver's license
• School or municipal tax account
• Act of purchase of the residential property that indicates the name of the owner

• Proof of affiliation to a Quebec professional association
• Statement of bank account in Quebec
• Credit card statement
• Notice of assessment from Revenu Québec

• Child's birth certificate (large format including first and last names of children parents)
• Change of name certificate
• Certificate of adoption judgment
• Confirmation of adoption judgment
• Placement order (if the place and date of birth of the child do not appear on the document, it is necessary to provide a complementary document where these data appear)
• Certificate of Indian Status
• Letter attesting the identity of the student and signed by the management of the reception center for the student in a reception center

对于不会说法语、刚来到加拿大、或使用临时移民身份的学生。要为孩子注册学校，家长必须致电514-596-6084或致函accueil@csdm.qc.ca 预约。

办公室位于：3737 Sherbrooke Street East (Joliette metro station)

营业时间：周一至周四，上午8:30至下午3:30。该服务于2016年12月23日至2017年1月8日期间关闭。
